Visa and Passport Updates
	September 6: The U.S. Passport Agencies and most consulates are CLOSED in observance of Labor Day. This day will not count in our processing times.

Passport Forms

All Forms are in Adobe Acrobat format.

DS 11: This form is used for child applications, Adult new passports and Lost/Stolen applications.

DS 19: This form is used for name changes, adding pages to your passport and extending the passport validity.

DS 64: This form is used with the DS-11 for lost/stolen US passport applications.

DS 82: This form is an application to renew an adult passport issued within the past 15 years.

DS 3053: This form is a statement of consent for a parent who cannot be present when applying for a child's passport.
