• who are applying for a passport for the first time OR
• whose prior passport was issued at age 15 or under OR
• whose prior passport was issued more than 15 years ago OR
• whose expired passport is not in their possession OR
• whose current, valid passport is damaged.

• who were 16 years old or older  when your most recent passport was issued AND
• the passport was issued less than 15 years ago AND
• the passport is still in your possession and is not damaged.

• who were 16 years old or older when the lost / stolen passport was issued AND
• Your missing passport would still be valid today (Passports are usually issued for 10 years).

• Your name has changed and you need to amend the passport with the updated legal name AND
• Your current passport is valid AND
• Your current passport has not been damaged.

• who are 15 years old or younger as of today AND
• You have not lost a US passport that would be currently valid.

• whose current passport is valid AND
• Your current passport has not been damaged in any way.
